  4. We have applied for Certificate of Confidentiality issued through the National Institutes of Health (NIH) to help protect your privacy. This certificate prohibits the disclosure of sensitive and identifiable information to anyone outside of the research team unless you consent to the release of that information, with the exception of a few specific situations that are described in the Informed Consent document.
